Notification of Academic Position Opening (NAPO)

  1. Submit the following to the Dean’s Office:
    • NAPO form located on the Human Resources ‘Recruitment’ page. NOTE: For open positions that would involve campus visits, please use the combination NAPO/Campus Visit form (See instructions and required supporting materials on Human Resources ‘Recruitment’ page.)
    • Position description and advertisement
    • Search committee list
    • Search plan
  2. Dean’s Office forwards paperwork to Affirmative Action for their signature.
  3. Affirmative Action sends to Provost’s Office for final approval. Affirmative Action needs list of search committee (name, race of each member when possible, and list of major recruitment resources)
  4. When approved NAPO is returned, the department may place an advertisement and open the search

Request for Authorization for Campus Visit

Submit the following to the Dean’s Office at least two weeks in advance of proposed visit:

Positions with indefinite tenure — call Dean’s Office

Request to Offer Academic Staff Appointment

Submit the following to the Dean’s Office:

New Appointments

              • Original of the appropriate Request to Offer form printed from PWAAPPT in Banner
              • Original plus copy of the Affirmative Action Compliance Statement, including required supporting materials
              • Two copies of the curriculum vita
              • For tenure-related appointments, please submit two copies of the offer letter sent to the candidate.

Renewal Appointments

              • Original of the appropriate Request to Offer form printed from PWAAPPT in Banner
              • One copy of the signed Affirmative Action Compliance Statement from previous appointment
              • Submit current vita every fourth year for annual appointments

NOTE: No Affirmative Action paperwork is required for renewals that have the statement “DO NOT ROUTE TO AAEO” in the AAEO box on the appointment tab in PWAAPPT. Submit the original and one copy of the Request to Offer Academic Staff Appointment to the Dean’s Office.

Courtesy Appointments

New or Renewal

(Include a statement in the “Remarks” section of the Unpaid Appointment Form outlining the individual’s proposed service to or connection with the department, division, and university.
              • One current curriculum vita (no more than three years old)

Sabbatical Leave

If leave plans are firm: Submit one original of the Sabbatical Leave Application and Contract form.

Leave dates should conform to academic term dates as listed at the bottom of the form.

Attach the following documentation:

              • statement of the purpose of the sabbatical leave
              • current curriculum vita
              • description of current teaching, research, and other professionally relevant activities

NOTE:  A faculty member will not be placed on Sabbatical Leave without the submission of a PRF to payroll. You may wish to wait until the faculty member has approval for Sabbatical Leave before submitting a PRF.

If leave plans are not yet firm:

Submit one original of the Request for Sabbatical Leave (Preliminary) form. Leave dates should conform to academic term dates. When leave plans become firm, the Sabbatical Leave Application and Contract form must be submitted with the documentation listed above.

Request for Leave of Absence

For both contingent and firm requests:

Submit one original of the Request for Leave of Absence form. Leave dates should conform to academic term dates.

NOTE: If a contingent request for LWOP (leave without pay) has been submitted, paperwork for a firm request must follow as soon as plans become definite. New paperwork must be filed when plans change after requests for leave have been submitted and/or approved. Forms should clearly indicate that it is a revision of a previous request. Also note that a faculty member’s pay will not be adjusted without the submission of a PRF to payroll. You may wish to wait until the faculty member has approval for the leave before submitting a PRF.

Faculty Reviews

Untenured Faculty Annual Reviews (May 1st)

Submit one copy of the following:

              • Department head’s review (be sure faculty member signs)
              • Written response from faculty member (if they submit one)
              • Faculty member’s statement of professional activities
              • Current curriculum vita

Untenured Faculty Mid-term Reviews (May 1st)

Submit one copy of the following:

          • Department head’s evaluation/committee review (be sure faculty member signs)
          • Faculty member’s statement of professional activities
          • Current curriculum vita
          • If contract is ending, submit two copies of Request to Offer Academic Staff Appointment form
